The second box can leave it blank
The third box is "subject"
The fourth big box in the middle is "content"
The last box of URL also can leave it blank
After that, press "投犒".

I also believe that this message should be written by Real VanNess because our king left the writer's name (投稿者) in "THANK YOU" while Subject (題名) is "Van Ness"......totally left the wrong position in the message box.  


Chinese & Japanese could fully understand how to leave the message because most of the Japanese words are in Chinese character.   Even Chinese could not understand Japanese, we could guess it.  

However, the mesage left by "VanNess" are left the wrong box.  So, I believe this message is really written by our king.
